The game functions as a direct sequel to Clouds of Xeen and concludes a narrative arc that began in the first title. It is a first-person dungeon crawler set in a fantasy world with science fiction elements, featuring turn-based gameplay.

Players control a party of up to six adventurers who travel to the Darkside of the world Xeen. Their objective is to investigate the disappearance of Prince Roland and confront Lord Alamar, a usurper who has overthrown the local queen. The party receives powers from the Dragon Pharaoh, the Guardian of Darkside, to aid in liberating the land and ultimately assisting the space traveler Corak in a larger conflict. Progression involves exploring towns, wilderness areas, and multi-level dungeons from a first-person perspective. Key gameplay systems include turn-based combat against various monsters, solving environmental puzzles within dungeons, and managing character advancement through experience and equipment. The game world contains pyramids that serve as teleportation devices.

The game utilizes the same engine and interface as Might and Magic III: Isles of Terra, making its core mechanics nearly identical to its immediate predecessor, Clouds of Xeen. Darkside of Xeen places a greater emphasis on its narrative compared to earlier series entries, incorporating a larger number of story cutscenes. Its setting is notably darker and more surreal. A unique feature is its direct integration with Clouds of Xeen; when both games are installed, they merge into a single larger campaign called World of Xeen.
