The game is a western adaptation of the Japanese title Nekketsu Kōha Kunio-kun, with its visual style and settings drawing noticeable inspiration from the 1979 film The Warriors.

Players control a protagonist who must fight through urban environments to rescue his kidnapped girlfriend. The adventure is divided into five distinct levels, which progress through locations such as subway stations, docks, and city streets. The core gameplay involves navigating these areas from a side perspective and engaging in hand-to-hand combat against numerous adversaries that appear on screen.

The combat system offers a variety of offensive maneuvers, including punches, kicks, headbutts, and flying kicks.
