The game presents a story where the martial artists Billy and Jimmy Lee, returning from training, are warned by a fortune teller of a great evil in Egypt and learn of the power of the Rosetta Stones.

Players control Billy and Jimmy Lee, with an option for a third player to control a new character named Sonny. The objective is to fight through linear stages of enemies across various locations, culminating in a confrontation in Egypt. The core gameplay involves moving from left to right, using a basic punch and kick combat system to defeat waves of opponents. New mechanics include the ability to run by quickly tapping the joystick twice and to perform cooperative moves with another player, such as a combined hurricane kick or a triangle jump kick. The game also introduces weapon shops where players can spend collected currency to purchase temporary power ups, extra lives, or special techniques.

Progression is stage based, with players advancing after clearing each area of enemies. The power ups purchased from shops can temporarily enlarge the characters, increasing their attack damage and range. The visual style shifted from the more cartoonish look of earlier titles toward a more realistic depiction of character sprites.
