Total Carnage is a top-down shoot 'em up released in 1994 for MS-DOS. The game was developed and published by Midway Games, Inc. Players control one of two characters, Captain Carnage or Major Mayhem, in a mission to confront the dictator General Akhboob in the fictional nation of Kookistan.

The player navigates through twenty distinct battlezones, moving in eight directions to engage enemies. The primary objective is to rescue hostages while eliminating waves of mutant adversaries. Progression is linear, with each area culminating in a confrontation against a large boss enemy that features a specific weak point. A standard machine gun is always available, but players can collect temporary power-ups to access other weapons such as flamethrowers, rocket launchers, and grenade launchers during a stage.

Gameplay focuses on constant shooting and evasion from an overhead perspective. The game supports simultaneous play for two players. Between the main combat stages, occasional mini-games appear, which require rapid button inputs to complete specific narrative actions. The setting involves a post-war landscape filled with radioactive mutants created by the antagonist's army.
