The game is presented from a top-down perspective and places the player in the role of manager for the titular team, Brutal Deluxe, a poorly performing squad in the fictional sport of Speedball.

The player's primary objective is to manage the team through a league system featuring two divisions of eight teams, with the goal of advancing to the top division. Additionally, two cup tournaments are available, one of which uses a two-leg aggregate score format. Management extends to playing the matches, which form the core interactive experience. Matches consist of two 90-second periods of fast-paced play where controlling a team of players to score goals is the aim. Gameplay encourages physical tackling to gain possession of the ball. On the pitch, various power-ups and tokens appear, some of which temporarily enhance a player's team or freeze opponents.

Progression is tied to financial rewards earned from match results and from collecting silver tokens that randomly appear during play. Between matches, this money can be spent on upgrading the abilities of existing players or on purchasing new, superior athletes to join the squad. The original team members have limited potential for improvement, making squad management a key consideration. The playing field includes several interactive elements, such as score multipliers on each side that increase goal values when the ball is passed through them, though opponents can negate this advantage. Five stars on the pitch can be hit for points, with their final values set at halftime, and portals that redirect the ball across the field add a further layer of tactical play.
