The player controls Leonard, whose objective is to rescue his prized pig, Bessie, after she and many of the town's residents are abducted by aliens. These aliens have cloned the local inhabitants, creating hostile duplicates that Leonard must fight. Progression involves navigating through a series of levels set in locations such as trailer parks and chicken processing plants. Gameplay centers on first-person combat using an unconventional arsenal, which includes weapons like a circular saw launcher, a TNT crossbow, and a machine gun bra. Instead of traditional health packs, the character restores vitality by consuming items like whisky and pork rinds found throughout the environments.

A notable gameplay system involves managing the character's condition through these consumables. Drinking whisky increases health and toughness, but excessive consumption triggers a temporary drunk mode that impairs vision and movement.
