The narrative centers on the infiltration of a fortified nation known as Outer Heaven, which is developing a prototype nuclear-capable walking tank called Metal Gear.

Players assume the role of Solid Snake, a new recruit in the elite FOX-HOUND unit. The primary objective is to infiltrate the enemy fortress, rescue a captured comrade named Grey Fox, and gather intelligence on the Metal Gear weapon. Progression involves navigating a multi-building complex with several floors and basement levels, using collected keycards to access new areas. Gameplay systems emphasize stealth, requiring players to avoid visual contact with enemy guards and patrols. Players start unarmed but gradually acquire various weapons and equipment, including a handgun, submachine gun, grenade launcher, and explosives like plastic charges and landmines, each with limited ammunition that must be replenished from found supplies. A suppressor attachment allows for quieter firearm use, and weapons can also be used to destroy certain environmental obstacles.

The structure includes confronting boss characters, who are elite mercenaries stationed within the base. Players can also locate and rescue hostages hidden throughout the buildings; rescuing hostages increases the player's rank, which in turn expands the life bar and carrying capacity for items. Communication is handled via a wireless transceiver, used to receive mission updates from commanding officers or to contact covert resistance members for tips. Enemy encounters can be handled through direct combat or by using stealthy melee attacks, with defeated guards sometimes dropping useful items if they are subdued without raising an alarm. The player's rank can decrease if a hostage is killed, adding a consequence to the rescue operations.
