The player controls the same protagonist from Hexen, who is transported to a realm of the dead after obtaining the Chaos Sphere. The primary objective is to navigate this new domain, reach the central citadel, and defeat the entities known as the Death Kings to secure an escape. Progression is structured across three new hub levels, which contain a total of twenty new single-player stages. Gameplay involves exploring interconnected environments, solving environmental puzzles, and engaging in combat using the character's established arsenal of weapons and magic against returning enemy types.
