The player controls a protagonist who ventures into an area known as the Slob Zone, which is controlled by an army of oversized animals called the Hardcore Union of Radical Litterbugs. The primary objective is to reclaim the world's stolen clean underwear from the leader, Bob the Slob. Instead of conventional combat, the core gameplay involves cleaning the hostile animals by using items like soap, water balloons, and deodorant. These cleaning supplies can be found in the environment or acquired from vending machines by trading collected rubbish.

Progression involves navigating through the Slob Zone while animals attempt to hit the hero with various kinds of dirt. The player must respond by using their cleaning utensils on the animals repeatedly until they surrender. This system of item collection, trading, and persistent cleaning against enemy assaults forms the central loop of the experience, defining the game's unique approach to action within a first-person perspective.
