Players control Cosmo as he navigates through a series of distinct environments, including alien tropics, a haunted forest, ice caves, and a futuristic world. The primary objective is to guide Cosmo through these lands to locate his missing parents. Progression is structured across three episodes, with each episode concluding at a specific story point. Gameplay involves running and jumping across platforms while avoiding or defeating various enemies. Cosmo can defeat foes by jumping on them or by using a limited supply of bombs. These bombs can also be used to uncover hidden secrets within the levels. Additionally, Cosmo is equipped with suction cup arms, which allow him to climb certain walls to reach higher areas or alternate paths.

The narrative unfolds across the three episodes. The first episode ends with Cosmo being swallowed by a large creature. The second episode takes place inside the creature's body, where the goal is to find an escape route. By the end of this episode, Cosmo discovers a city where he believes his parents may be held. In the third and final episode, Cosmo is reunited with his parents and the story concludes.
