The game follows the character Billy Blaze, also known as Commander Keen, who finds himself in a dream world called Tuberia, a land ruled by sentient vegetables.

Players control Commander Keen as he navigates a series of discrete levels. The game begins with an overhead map of Tuberia, which serves as a level selection screen where Keen can walk to choose his next destination. The primary objective is to progress through each side-view stage, ultimately aiming to confront the antagonist, King Boobus Tuber. To reach this goal, Keen must first locate a specific number of Boobus Bombs scattered across the levels, as these are required to defeat the king. Progression involves moving from the start of a level to its exit, while avoiding environmental hazards and hostile enemies.

Gameplay focuses on exploration and avoidance, as Keen lacks his traditional raygun and pogo stick from other series entries. Instead, he uses a weapon called Flower Power, which are bombs that can be thrown to temporarily transform enemies into harmless flowers. These bombs can be retrieved if they miss their target and land on the ground. Throughout the levels, players can collect various items, including keys to unlock doors, point bonuses, extra lives, and a rare magic eyeball that grants additional lives and ammunition. Keen loses a life if he is touched by an enemy, falls into a pit, or contacts any hazardous surface.
