The narrative follows the young protagonist Billy Blaze, also known as Commander Keen, who must travel to the planet Fribbulus Xax after his babysitter Molly is captured by the alien Bloogs.

Players control Commander Keen across a series of interconnected levels. The adventure begins on a top-down map of the planet, which serves as a level selection screen. From this map, the player chooses which area to enter next, though some paths are initially blocked and require specific items to access. Each individual level is a side-scrolling platforming environment where the primary objective is to navigate from the starting point to an exit. Completing a level returns Keen to the overhead map to plan the next stage of the journey.

Gameplay involves navigating platforms while avoiding environmental hazards and hostile creatures. Keen is equipped with a raygun to stun most enemies, which include club-wielding Blooguards, reflective Flects, and charging Nospikes. He also uses a pogo stick for enhanced jumping ability to reach higher areas. Throughout the levels, players can collect various items, including ammunition for the raygun, gems used to open locked doors, and score-boosting bonus items. Collecting one hundred bird-like Vivas grants an extra life, as does finding the rare Queen Viva. Contact with enemies or environmental dangers results in the loss of a life.
