Players control Caleb, a resurrected gunslinger seeking revenge. The game is structured into four episodes, each composed of multiple maps that must be completed in sequence. Levels are designed as coherent locations such as a carnival, a city district, or a moving train. Progression involves locating keys, activating switches, and fighting through enemies to reach an exit. Core gameplay systems include exploration of these detailed environments and combat against a variety of foes, including zombies, cultists, and supernatural creatures. Caleb employs an arsenal of unconventional weapons, ranging from a pitchfork and sawed-off shotgun to a tesla cannon and a magical staff. Enemies drop "Life Essences" to restore health, rather than standard medikits. Players can also collect different types of armor for specific damage resistance and various power-ups with instant or delayed effects. The game features intense and graphic violence, with detailed enemy death animations and environmental gore. Each episode concludes with a boss encounter, challenging the player's mastery of the game's weapons and systems.
