The player guides Baby Jo through four distinct levels, moving from left to right across the screen. The primary objective is to avoid or defeat various hostile creatures and obstacles, such as bees, lawnmowers, and spiders. To defend himself, Jo can pick up rattles scattered throughout the stages and throw them at enemies to temporarily disable them. Progression requires careful timing and platforming to navigate the terrain while managing several character status indicators.

Gameplay involves monitoring multiple survival metrics for the baby character. A health meter is represented by Jo's face, which becomes increasingly tearful as he takes damage. Additionally, the baby's diaper gradually becomes wetter over time, and a separate thirst meter increases. If either the diaper becomes completely soaked or the thirst reaches a critical level, the player immediately loses a life. Therefore, a key part of the game is collecting fresh bottles of liquid to reduce thirst and clean diapers to reset the wetness meter, alongside the standard platforming challenges.
